We are excited to announce our panelists who will share their work on social determinants of health: Dr. Kathryn Parke (UT School of Nursing) and Ms. Tiffany Gonzalez (Bluebonnet Trails Community Services).

Please let us know which breakout session you plan to attend by making your choice here. You will also have the option to indicate if you are unable to stay for the breakout sessions. We have two sessions planned. You can read more about them here.

San Antonio Food Bank Food and Funds Drive – We are very excited to partner with the Omni Hotel and the San Antonio Food Bank to collect food and funds for our neighbors in the region and those impacted by Hurricane Harvey. The employees of the Omni will be donating throughout the week and we will wrap up the collections at our Learning Collaborative. Thank you in advance for giving generously! Updated Community Needs Assessment – As part of this year’s DSRIP requirements, and in preparation for the transition to Demonstration Years 7 and 8 (if approved by CMS), Regional Healthcare Partnerships are required to update their Community Needs Assessments. We are pleased to share with you RHP 6’s draft Community Needs Assessment Update for DY6. We need your input! We invite you to review the draft and share your feedback and insights by completing this survey. The survey will close October 31, 2017.
