Readmissions Learning Collaborative with TMF Quality Innovation Network

RHP 6 is partnering with the Texas Medical Foundation (TMF) Quality Innovation Network to continue our efforts to improve readmission rates through care coordination, medication safety, and cross-sector collaboration.

DY6 Road Trip: Social Determinants of Health

On December 9, 2016, RHP 6 and Hill Country Memorial Hospital held a DY6 DSRIP Road Trip. We discussed the “other 80%” of things that impact health outcomes.

According to research, clinical care may only contribute 20% to health outcomes. During this event, we heard from providers and other organizations who are proactively helping patients secure healthy foods, supportive housing, and access to vital social services.

DY5 Sustainability Retreat Learning Collaborative

On Tuesday, May 10, 2016, RHP 6 hosted a Sustainability Retreat Learning Collaborative. The purpose of this event was to celebrate another successful round of DSRIP reporting and provide an opportunity for DSRIP providers to work together to brainstorm, develop, and document plans related to program evaluation (especially logic models) and sustainability action planning. This event followed six months of online learning and collaboration through the ACES webinar series.

This was an informal and highly participatory “working session”  with very limited didactic presentations. Well over half of the event time is allotted for participants to work alongside their peers to develop project-specific logic models and sustainability action plans. In the spirit of our “retreat” theme, casual attire (jeans, yoga pants, tshirts) is encouraged!

Intended audience: DSRIP project managers, organizational DSRIP “champions” and team leads.
